Adult Medicaid and Long Term Care
Adult Medicaid and Long Term Care applications must be made in person by you or your representative at the 300 N. Duke Street office between 8:15 am and 5:30 pm Monday through Friday except county holidays. Bring any bills or information you feel you will need in order to apply.
Private Living Medicaid
Private Living Medicaid is for an aged, legally blind or disabled person over the age of 21 who is able to live at home and care for themselves.
Long-Term, Rest-Home & Assisted Living Care
Long-Term Care is for a person who lives or resides in a nursing home or assisted living facility.
Rest-Home or Assisted Living care is for a person who needs assistance with activities such as preparing meals, housekeeping and perhaps dressing and bathing.
Family and Children Medicaid
Medicaid for Families and Children is located at 300 N. Duke Street and accepts applications in person from 8:15 am until 5:30 pm Monday through Friday except county holidays.
