   Health Check and NC Health Choice
Two programs in North Carolina are now available to help children get the insurance they need.  Health Check (Medicaid) provides free health insurance to children aged 0 through 20 whose families qualify.  NC Health Choice provides free or low cost health insurance to uninsured children aged 0 through 18 whose families cannot pay for private insurance and who do not qualify for Health Check (Medicaid).

What are the benefits?
Eligible children will receive coverage for:
 
Well Child Checkups Sick Visits Lab  Tests Hospital Care
Medicines Dental Care Counseling Therapies
Vision and Hearing Care Medical Equipment & Supplies Immunizations (Shots) Surgery

Additional benefits may be available for children with special health care needs. 

How do I know if my child is eligible?
Your child may be eligible for one of these programs if your family size and income (before taxes) is less than:

Some eligible families, depending on income, may have to pay an enrollment fee and co-payments for their child's care.  The fee is $50-$100 per year.  Many families do not pay any fees at all.
  

How do I apply?
To apply for Health Check of NC Health Choice, a person completes one application form.  Application forms are available at the following locations or can be downloaded

Families need to bring or mail completed applications, along with proof of income for the past month (wage stubs) and their child's age, to the department of social services in the county where they live..

How do I know if my child is covered?
Your local department of social services reviews all application forms.  DSS will inform you by mail or phone if your child qualifies for a program.  They will also tell you if you have to pay for an enrollment fee.  Children are enrolled in either program for one year.  Families must reapply each year.
