| The Health Network is a collaboration between Duke University Medical Center, Durham County Social Services and the Durham County Health Department.
When clients sign up for Medicaid through Carolina Access, they have the option of choosing the benefits of the Community Care of North Carolina Program, administered locally in Durham through DCHN. At the time they enroll, our clients may choose a primary care provider who will manage their overall healthcare needs. If they need specialty care, the provider arranges for the client to see a specialist or receive other health services, such as nutrition counseling. Health Network social workers provide care management for Medicaid patients at risk or suffering from diabetes, asthma or hypertension.

Local Access to Coordinated Health Care (LATCH) is a program to connect families to medical coverage and care. Services are available in English and Spanish.
In 2002, Duke Division of Community Health, in collaboration with multiple Durham organizations, launched Local Access to Coordinated Healthcare (LATCH) to address the increasing numbers of Durham County residents who lacked health insurance, a regular source of care or “medical home,” and for whom accessing the healthcare system was a growing challenge. The County’s fast-growing immigrant community, most notably newly arrived Latino immigrants, was overwhelmingly uninsured and faced unique language, cultural and financial barriers when accessing the healthcare system. The impact of these obstacles on health was evident in many healthcare realms, especially local hospital emergency departments where uninsured patients sought medical care for conditions that included untreated diabetes, hypertension, asthma, and depression, and pregnant women showed up to deliver a baby with late or no prenatal care.
